Description: This dataset represents hospices holding "Class A" license in the state of Oklahoma. Hospices class A are Medicare certified (See 2011 Annual Hospice Report). The list was provided by and geocoded the Health Care Information Division at the Oklahoma State Department of Health (OSDH). The data were geocoded at Tier 3B, 3C ACOG, ESRI11 and 4D (see OSDH Geocoding Standards) by address using the 2012 NAVTEQ NAVSTREETS, ACOG, ESRI data and online resources. The address used for geocoding was the physical address of the facility as reported by the medical facility service and Health Care Information. The data are current as of December 2013. Please contact the originator regarding errors or omissions.
